Necessary Reports for the City

When a child residing in Inagi City is admitted, please have the Parent or Guardian fill out the "Usage Report". In the case of discharge, please have the Parent or Guardian fill out the "Usage Termination Report" and submit it promptly to Inagi City through the facility.
If there are children residing in Inagi City as of April 1 each year, the submission of a "Usage Status Report" is required. Please submit it to Inagi City by April 30 of each year.
