This is a system where you register your address, medical history, emergency contact information, etc. with the fire department in advance, and by providing the registration number when calling an ambulance, the emergency team can respond to the registered address with the caller's information understood.

How to Join

In addition to phone and in-person visits, you can complete the enrollment process through the electronic application form.
Support is also available at the Hirao Branch Office Fire Station.

Start Date of Use

After receiving your application, we will send a card with a number printed on it by mail in about one week. The card will be usable as soon as it arrives.

Notes

1. When an ambulance is dispatched to the registered address, please inform us of your current location as usual when reporting from outside your home.
2. If there are any changes to your registration details (such as moving, changing your primary care provider, or death), please contact the Inagi Fire Station promptly.
3. Since it is necessary to regularly check information such as relocations, there may be a verification of the registered information in the Basic Resident Register and the emergency medical notification registration information.
4. Applications are generally to be submitted by the individual themselves; however, if it is difficult for the individual to apply for some reason, it is necessary to submit the following "Appointment of Agent Notification".
