Re-registration Procedures for Individuals Requiring Evacuation Assistance
The other day, I received a letter from the Welfare Department Livelihood Welfare Division Community Welfare Section requesting the re-registration of the evacuation support registration card.
Since I qualify as a target person, I have been registered for some time, but unfortunately, the registration form I received this time was completely blank and did not reflect any of my previous information. Due to my severe difficulties with writing, I struggled greatly to finally complete the form.
Such issues can be easily resolved using IT technologies, such as creating forms on the internet.
If you are requesting applications or registrations for people with disabilities, could you please provide an easy way to submit, taking into account the diverse realities of disabilities?

We apologize for the inconvenience caused during your registration. We have now started accepting applications (new and changes) via a form on the city website. Please use that from now on.
