Issuance of COVID-19 Vaccination Certificate (Vaccine Passport)
After April 1, 2024, the installation of the vaccination certificate app, the issuance of vaccination certificates through the app, and the issuance of vaccination certificates at convenience stores will no longer be possible.
For paper-based vaccination certificates, please continue to check the following "Application for Paper COVID-19 Vaccination Certificate" and apply accordingly.
In addition to the overseas certificates that have been accepted so far, you will now be able to apply for vaccination certificates for use within Japan. Please note that for use within Japan, the "Vaccination Certificate" and "Vaccination Record" can still be used as before. No new application is necessary.

The certificate will be issued and mailed within about one week from the date of acceptance.

- Application Form
- Copy of passport (for overseas certificate applicants only)
- A copy of the identification document with the return address (driver's license, Health Insurance Card, etc.)
- A copy of something that shows the vaccination ticket number (vaccination ticket or pre-vaccination questionnaire, etc.)
- Copy of vaccination certificate or vaccination record
- Reply envelope (with address written and stamp affixed)

- Documents for confirming maiden name, different surname, or alias (if the passport includes the maiden name, different surname, or alias, documents for confirming the maiden name, different surname, or alias)
- Power of Attorney for Claims by an Agent
Note: A power of attorney is required if someone other than the applicant is applying.
