Application for Funeral Services Expenses

If you are enrolled in Late-Stage Elderly Medical Care insurance and have passed away, a subsidy of 50,000 yen will be provided to the person who conducted the funeral services.

Required Items for Application

1. Funeral Invoice or Receipt
2. Documents that can confirm the transfer destination
3. Identification documents for those bringing the Application Form (My Number Card, driver's license, etc.)

Note: If you wish to transfer to an account that is not in the applicant's name, please submit a power of attorney.
