The Inagi City Home Medical Care and Nursing Care Coordination Map has been released.

In Inagi City, we are implementing the "Home Medical Care and Nursing Care Coordination Promotion Project" aimed at promoting collaboration among related organizations such as medical institutions and nursing care service providers, so that Older Adults who require both medical care and nursing care can continue to live their lives in their familiar community until the end.
As part of this initiative, we have created the "Inagi City Home Medical Care and Nursing Care Coordination Map" which summarizes medical institutions and nursing service providers in the four areas of Inagi City. We hope you find it useful.

Note: The information published was surveyed from October to December of 2022. The facilities listed are those that have agreed to be included in the "Inagi City Home Medical Care and Nursing Care Coordination Map."
