We are securing support beds for those receiving home medical care.

In Inagi City, we secure hospital beds that can be quickly accessed when home care becomes temporarily difficult for older adults in need of care and their families, allowing them to live safely in the community while receiving medical care at home.
Please consider this as a reference when contemplating the use of home medical care, and if needed, please make use of it.

(Note) In cases of consciousness disorders, difficulty breathing, body convulsions, etc., please call 119 instead of using this service.
In addition, if short-term admission under Long-Term Care Insurance is available, that will take priority.

Eligibility

Those who meet either 1 or 2 below

1. Residents of the city who have been certified as requiring support or care, or older adults equivalent to that
2. Individuals who are continuously receiving management and guidance from a physician providing medical care at home or healthcare professionals such as visiting nurses under the direction of a physician.

Usage Procedures

1. Please consult your primary care physician or visiting nurse.
   The primary care physician will check the condition of the person receiving home medical care and will apply to the hospital by phone.
   (Applications cannot be made directly to the hospital by the individual or their family.)
2. After the primary care physician submits the hospitalization application to the hospital, the user themselves must fill out the "Application Form for Inagi City Home Medical Care Support Bed Usage" (Form No. 1) and the "Basic Information Sheet and ADL Table for Inagi City Home Medical Care Support Bed Usage" (Form No. 2) (can be filled out by another person) and submit them to the hospital at the time of hospitalization.

Usage Period

Within 14 days from the date of hospitalization in principle
Note: If hospitalization treatment continues after this project, please follow the hospital's instructions.

Usage Fees

It is the same as a regular hospitalization.
Using the Health Insurance Card, the portion of the cost that is the user's responsibility and expenses not covered by health insurance will be borne by the user.
Transportation costs related to hospitalization will also be the responsibility of the user.
(Note) Please follow the instructions of the cooperating hospital regarding the use of hospital beds.
