Handbook of Love

This is a necessary handbook for individuals with intellectual disabilities to receive various support systems. Based on the guidelines for the issuance of the Love Handbook, it is comprehensively assessed based on factors such as intelligence measurement, social skills, communication, physical health, and basic living skills, and is issued in degrees from 1 (most severe) to 4 (mild). It corresponds to the national "Childcare Handbook."

Procedure

New Grant

To receive the issuance of a certificate, individuals under 18 years old need to be assessed at the Tama Child Consultation Center, while those 18 years and older must be assessed at the Tokyo Disability Welfare Center. Please make a reservation for the assessment directly by phone.

Update (Reassessment)

When the individual turns 3, 6, 12, or 18 years old (adult renewal), or when there is a significant change in the degree of disability, those under 18 should receive a reassessment at the Tama Child Consultation Center, and those 18 and older should receive a reassessment at the Tokyo Disability Welfare Center. Please make a reservation for the assessment directly by phone.

Changes, etc.

Applications for procedures other than assessment (such as changes to name, address, etc., reissuance, and refunds) can also be made at the Disability Welfare Division.
Note: Those moving from another prefecture need to not only change the address on their handbook but also undergo a determination for switching from the Child Development Handbook to the Love Handbook. For more details, please contact the Disability Welfare Division.
