Flow of Medical Examination

Flow of receiving an individual health checkup 1. Apply for the "examination ticket" at the Health Center 2. Receive the "examination ticket and information" from the Health Center 3. Make a reservation at the medical institution 4. Undergo the health checkup

Eligible Individuals

Citizens aged 40 and over (born on or before April 1, 1987)

Note: Those who have ever undergone a hepatitis virus test in the past are not eligible.

Note: For those born between April 2, 1986 and April 1, 1987, a medical examination ticket will be sent from the city. The application procedure for the examination ticket described below is not required.

Application Deadline

The deadline is the 22nd of every month, and the examination tickets will be sent out in early the following month.

Note: The final application deadline is December 22, 2026.

Process from Application to Examination

Please apply for the examination ticket at the Inagi City Health Center using one of the following methods: (1) (2) (3).

(1) Postcard or letter

Write the title 'Hepatitis Virus Screening Application', fill out the following, and mail it to the Inagi City Health Center.

  • Name (Furigana)
  • Date of Birth
  • Address
  • Phone Number

(2) Logo Form (Online)

(3) Fill out the designated form at the Health Center counter

Office hours = Weekdays from 8:30 AM to 5:00 PM (Closed on Saturdays, Sundays, holidays, and from December 29 to January 3)
