Application for My Number Card for Those Moving Abroad

Japanese nationals who moved abroad after October 5, 2015, can apply for the My Number Card for overseas residents.
The application method is as follows. For information on receiving, please see the following link.

Application Location

Please apply by visiting one of the following locations or by mail.

  • Overseas Diplomatic Missions
  • Municipality of Temporary Residence
  • Registered Municipality

Note: If you need to reissue your My Number Card due to damage or loss, you cannot apply by mail.

Required Documents [New Application / Renewal Application]

  • Application Form for Issuance of Individual Number Card and Application for Issuance/Update of Electronic Certificate (Attach a passport-sized photo)
  • Request Form for Setting PIN for Individual Number Card and Electronic Certificate
  • Note: If a voluntary agent is applying, after the principal has filled out the Application Form for setting a PIN, please place it in an envelope, seal it with glue or similar, and bring it in a state where the agent cannot see the PIN.
  • Note: Please check the specifications for the photo at "Face Photo Checkpoints (My Number Card Comprehensive Site)".
  • Note: Please download the Application Form from "[Overseas] Application Form Download (My Number Card Comprehensive Site)".

  • Application Form for Reissuance of Individual Number Card and Issuance/Update of Electronic Certificate (Attach a passport-sized photo)
  • [For those who have lost it] Documents proving the fact that the My Number Card has been lost (such as a lost property certificate)
  • For those who have damaged cards: Damaged or lost My Number Card
  • Note: If the document certifying the fact of loss is issued in a foreign country, please attach a Japanese translation.
  • Note: Please check the specifications for the photo at "Face Photo Checkpoints (My Number Card Comprehensive Site)".
  • Note: Please download the Application Form from "[Overseas] Application Form Download (My Number Card Comprehensive Site)".

Fee

Free
However, a fee of 1,000 yen (800 yen if no electronic certificate is included) will be charged in the following cases.

  • Reissue due to loss, damage, contamination, or burning of My Number Card
  • Reissuance of My Number Card after voluntary return, etc.

For those whose application location is at a diplomatic mission or temporary residence, and the receipt location is at a diplomatic mission (if Inagi City is the registered domicile)

As soon as the documents such as the application form are confirmed in Inagi City, we will inform you about the payment method via email.
If you are a resident of Inagi City, please make your payment using either method 1 or 2.
Note: Since payment methods vary by municipality, those with their registered domicile in other cities or towns should check with their registered domicile.

  1. Direct payment at City Hall (Payment at Inagi City Hall by relatives or friends residing in Japan)
    Please inform us of the name and date of visit of those who will come to the office in advance.
  2. Cash Mailing (Cash on Delivery / Fixed Amount Postal Money Order)
    • If you are sending cash, please send it in Japanese yen (please ensure there is no change).
    • If you are sending a fixed amount postal money order (Japanese), please send one that was issued within the last 6 months.

Note: Please do not write any names or other information on the fixed amount postal money order. Additionally, payment by stamps is not allowed.

  • Please do not fill in any information such as your name on the fixed amount postal money order.
  • Payments in local currency are not accepted.
  • Payment by stamps or similar is not accepted.
  • Refunds for fees are not possible.

Period from Application to Receipt

The period from application to issuance of the My Number Card is approximately 2 months (as a guideline). If you do not receive the notification email after more than 3 months from your application, please contact your registered domicile.
