These are the forms to be submitted when registering or deregistering motorized bicycles and small special vehicles.

Forms for the Application Form for Issuance of Plates and the Plate Return Form are also available at the counter, so you can fill them out when you visit.

Please bring the Transfer Certificate and Power of Attorney that have been prepared in advance by the previous owner or the person granting the power of attorney.
