If you have lost your sign issuance certificate, you can now apply for reissuance by mail without having to visit the counter. (There is no fee for reissuance.)
If you wish to request reissuance by mail, the following documents are required. Please note that reissuance may be delayed if there are any deficiencies or missing documents.

Required Documents for When the Owner Applies

  • Copy of identification documents (driver's license, Health Insurance Card, My Number Card, etc.)
  • Application Form for Reissuance of Light Motor Vehicle License Certificate (Please download the PDF file below, print it out, fill in the necessary information, and enclose it.)
  • Reply envelope (limited to those with the recipient's address written and postage affixed)

Required Documents for Application by an Agent

If an agent is applying, in addition to the three necessary documents mentioned above, the following documents will be required.

  • Power of Attorney (Required even for applications by relatives in the same household.)
