Paper Health Insurance Cards for National Health Insurance will no longer be newly issued from December 2, 2024

In Japan, nationwide, from December 2, 2024, the system has shifted to basically using the My Number Card as the Health Insurance Card, and the issuance of Health Insurance Cards has ended.

  • Note: Receiving medical treatment with the My Number Card is limited to medical institutions, etc., where card readers are installed.
  • Note: A "first-time registration" is required for the use of the Health Insurance Card through My Number Portal, etc.

Regarding Measures from December 2, 2024

Those who have registered to use the My Number Card as a Health Insurance Card should use the My Number Card as their Health Insurance Card. In addition, a "Qualification Information Notice" (A4 size) will be issued at the time of enrollment procedures, etc.

Those who have not registered to use the My Number Card as a Health Insurance Card will be issued a "Qualification Confirmation Document" (card type) at the time of enrollment procedures, etc. By presenting the "Qualification Confirmation Document" at medical institutions and other reception desks, you can receive insured medical treatment.

Notification of Qualification Confirmation and Qualification Information

Qualification Confirmation Document

The Qualification Confirmation Document is issued to those who have not registered to use the My Number Card as a Health Insurance Card, and by presenting it at medical institutions or other service counters, they can receive medical treatment.
Qualification confirmation documents cannot be issued in principle to those who have registered to use the My Number Card as a Health Insurance Card.

However, those who are unable to use the My Number Card as a Health Insurance Card due to age, disability, etc., can apply for the issuance of a qualification confirmation document by submitting an Application Form.

Notification of Qualification Information

The notification of qualification information is intended to make it easy to understand the insured person's qualification information. It will be issued to those who have registered to use the My Number Card as a Health Insurance Card.
The notification of qualification information alone cannot be used to receive medical treatment at medical institutions, etc. Please use the My Number Card as your Health Insurance Card.
However, if the My Number Card cannot be read at medical institutions, etc., you can receive medical treatment by presenting both the My Number Card and the notification of qualification information.

Also, since the "Notification of Qualification Information" may be requested for presentation in administrative procedures, please keep it stored safely.

Other Certificates

Certificate of Eligibility for Maximum Amount

Issued to those who have applied.
If you use your My Number Card as a Health Insurance Card and agree to use the high-cost medical expense system, the certificate of eligibility for the maximum amount will not be required (in principle, no application is necessary).

Specific Disease Treatment Certificate / National Health Insurance Recipient Certificate

A paper certificate will be issued. Even if you use your My Number Card as a Health Insurance Card, please also present the paper medical certificate.

Benefits of My Health Insurance Card

You can view "Specific Health Checkup Information" and "Medication Information" on the My Number Portal

You can view your specific health checkup information and medication information on the My Number Portal.
Also, if you give your consent, information about specific health checkups and the medications you have taken in the past will be accessible to medical institutions.
The quality of medical care improves by being able to provide examinations and prescriptions tailored to your information.

Procedures for temporary payments exceeding the limit amount will no longer be necessary at medical institutions, etc.

If you have registered to use your My Number Card as a Health Insurance Card and use medical institutions where online verification of medical insurance eligibility is possible, you will not need to present documents such as the "Certificate of Application of the Limit Amount".

Medical expense notification information can be linked via data when preparing the tax return.

You can also check Medical Expenses notification information on My Portal. In principle, information for medical services from two months prior is updated on the 11th of every month.
However, there are also costs for treatments such as judo therapy, acupuncture, and massage that are not displayed.

The use of the Health Insurance Card requires initial registration.

To use the My Number Card as a Health Insurance Card, please note that you need to perform "initial registration" from the My Portal, etc.

  • Register from a smartphone compatible with My Number Card reading
  • Registration from a computer (Card reader required)
  • Register from Seven Bank ATM
  • Register from the My Portal terminal at City Hall (Please bring your My Number Card and visit the Insurance and Pension Division.)

Expiration Date of the My Number Health Insurance Card

With the My Number Health Insurance Card, the user certificate electronic certificate of the My Number Card (hereinafter referred to as the "electronic certificate") is used to verify eligibility at medical institutions, etc. However, since the validity period of the electronic certificate is until the fifth birthday after the issuance of the electronic certificate, it is necessary to complete the renewal procedure before the electronic certificate expires.

A "request" is required to cancel the registration of the Health Insurance Card.

If you wish to cancel the registration of your Health Insurance Card using the My Number Card, you need to submit a cancellation application to your health insurer.

Application Method for Those Enrolled in the National Health Insurance

Please fill out the above Application Form and submit it to the City Hall Insurance and Pension Division or the Hirao/Wakabadai Branch Office.

  • Note: If you cancel your registration, you will no longer be able to perform online eligibility verification using the My Number Card.
  • Note: After cancellation, you are required to bring your eligibility verification certificate when visiting medical institutions, etc.
  • Note: If you do not have an eligibility verification certificate at the time of cancellation application, one will be issued to you.
  • Note: After canceling the registration, it will take about 1 to 2 months for the changes to be reflected on the "Health Insurance Card Registration Application Status" screen on My Portal.
