From December 2, 2024, the system has been based on using the My Number Card as a Health Insurance Card, called the My Number Health Insurance Card.
Since the currently issued Health Insurance Cards and qualification confirmation documents will expire between the end of July and the end of September, 2025, qualification information notices have been mailed in July 2025 to household heads who have a My Number Health Insurance Card, and qualification confirmation documents have been mailed to those who do not have one. If there are members in the household who have a My Number Health Insurance Card and those who do not, the documents are sent separately by mail.
From August of Reiwa 7 onwards, please present either the My Number Health Insurance Card or the eligibility confirmation document (salmon color) mailed in July to medical institutions when receiving medical treatment.
Note: People aged 75 and over are covered by the Late-Stage Elderly Medical Care system (not National Health Insurance).

If you do not have a My Number Health Insurance Card, you can use the qualification confirmation document (salmon color) sent in July to visit medical institutions after the expiration date of your current paper Health Insurance Card (cream color) on October 1, 2025. If you do not have a My Number Health Insurance Card and have not received the qualification confirmation document, please contact us.
If you have a My Number Health Insurance Card, please continue to use your My Number Card when visiting medical institutions.
Validity Period of My Number Health Insurance Card, Paper Health Insurance Card, and Qualification Confirmation Document
The My Number Health Insurance Card does not have an expiration date, so it can be used as is.
The paper Health Insurance Card is valid until September 30, 2025, in principle.
The paper qualification verification document is generally valid until July 31, 2027.
From August 2025 onwards, those who do not have a My Number Health Insurance Card should present the eligibility confirmation document mailed in July to medical institutions when receiving medical care.

For those who have a My Number Health Insurance Card, a notification of qualification information will be issued.
For those who do not have a My Number Health Insurance Card, a qualification confirmation document will be issued.

If the My Number Health Insurance Card cannot be read by the medical institution's card reader, you can receive medical treatment by presenting this along with the My Number Health Insurance Card.

Qualification Confirmation Document
You can receive medical treatment by presenting it at the medical institution's reception desk, just like the current paper Health Insurance Card.

Are you enrolled in social insurance or the National Health Insurance Association?
If you are enrolled in social insurance or a National Health Insurance union, you need to complete the procedure to withdraw from the National Health Insurance. Please complete the procedure promptly.
About the Expiration Date
The expiration date is as stated on the ticket.
- For those whose period of stay expires by July 31, 2027 = Until the period of stay expires
- For those other than the above = Until July 31, 2027
Expired Health Insurance Cards, Qualification Confirmation Documents, and Qualification Information Notices can be discarded by yourself with attention to personal information.
For those aged 70 to 74
For those aged 70 to 74, the self-pay rate will be either 30% or 20% depending on income. Until December 2024, an Older Adults' Recipient Certificate indicating the self-pay rate was issued, but the My Number Health Insurance Card allows you to check the self-pay rate online, and the Eligibility Confirmation Document includes the self-pay rate, so when visiting medical institutions from August 2025 onward, please present either the My Number Health Insurance Card or the Eligibility Confirmation Document sent in July.
Expiration Date (For those aged 70 to 74)
The validity period of the qualification confirmation document and qualification information notice is until July 31, 2027. (For those who will turn 75 years old by July 31, 2027, it is until the day before their birthday.)
If there is a change in the burden ratio before the expiration date, a new qualification confirmation document and qualification information notice will be sent again.
Expired Health Insurance Cards, Qualification Confirmation Documents, and Qualification Information Notices can be discarded by yourself with attention to personal information.

- Even for those subject to a 30% burden, if the total income in 2024 of National Health Insurance members aged 70 or older is confirmed by the city to be less than 3.83 million yen for one person, or less than 5.2 million yen for two or more people, the burden will be 20%.
- For National Health Insurance members aged 70 and over, if the total "amount of income after basic deduction" is 2.1 million yen or less, the burden will be 20%.
