Integrated Implementation Project for Health Services and Care Prevention for Older Adults

Project Overview

 In fiscal year 2020, the "Act on Securing Medical Care for Older Adults" was amended, and a new system called "Integrated Implementation of Health Projects and Long-Term Care Prevention for Older Adults" was launched. Information from the three insurances (Late-Stage Elderly Medical Care, National Health Insurance, Long-Term Care Insurance) will be shared to comprehensively implement projects tailored to local health issues.

 In Inagi City, the Insurance and Pension Division and the Senior Welfare Division collaborate and work together with the Tokyo Metropolitan Late-Stage Elderly Medical Care Wide-Area Union to promote the "Integrated Implementation of Health Services and Care Prevention for Older Adults." Additionally, coordination and cooperation are carried out with the Inagi City Medical Association, Tokyo Hanan Dental Association Inagi Branch, Inagi City Pharmacists Association, and the Community General Support Center.

Initiatives of Inagi City

1. We will utilize data related to health checkups, medical care, and nursing care information to analyze local health issues and connect them to initiatives.

2. High-Risk Approach (Individual Support for Older Adults)

 Note: Individual guidance will be sent to the eligible persons. The visiting medical professionals will be wearing name badges.

(1) People taking multiple medications

Healthcare professionals visit to provide medication guidance and support, helping to prevent adverse drug events such as falls. They also identify potential health issues and prevent frailty related to aging. Services and appropriate agencies are connected as needed.

(2) Persons with unknown health status

For those who have not undergone medical or dental checkups and have not received nursing care certification, healthcare professionals will conduct home visits to assess their health condition. If necessary, they will encourage medical consultations, provide health consultations related to frailty prevention, and connect individuals to services and appropriate institutions.

3. Population Approach (Active Involvement in Community Spaces, etc.)

(1) At community centers, medical professionals will conduct outreach lectures (on frailty prevention, malnutrition, how to manage medications, etc.). Connections to services and appropriate institutions will be made as needed.

(2) For those who do not participate in the regular meeting places, we will widely recruit the general public and hold seminars (on the effects of polypharmacy, medication management, etc.).
