Examples of identification documents at the Taxation Division

One Identification Document (Category A)

Verification is conducted with one of the following documents.

• My Number Card (Individual Number Card)
• Driver's License, Driving History Certificate
• Passport
• Physical Disability Certificate, Mental Disability Health and Welfare Certificate, Ai-no-Techo, Rehabilitation Certificate
• Residence Card, Special Permanent Resident Certificate
• Other official identification documents issued by government offices recognized as equivalent, with a photograph displayed (e.g., Real Estate Transaction Specialist Certificate)

2 Confirmation Documents

Verification will be conducted using either "2 documents from Category B" or "1 document each from Categories B and C."

• Note: "2 documents from Category C" are not acceptable.

Category B

• Qualification confirmation documents for health insurance, etc. (National Health Insurance, Health Insurance, Late-Stage Elderly Medical Care, Mutual Aid Associations, etc.)
• Long-Term Care Insurance Certificate
• Pension Certificates (National Pension, Employees' Pension, Mutual Aid Pension, etc.)
• Public Assistance Recipient Certificate
• Other documents issued by government offices recognized as equivalent (such as tax payment notices)

Category C

• Identification issued by corporations other than government offices (such as employee ID cards)
• Student ID
• Other documents recognized as equivalent to these, containing the name (hospital patient card, cash card, credit card, bankbook, etc.)
