When applying for the issuance of a tax payment certificate, applying for account transfer, reissuing payment slips, or consulting about tax payments, we will verify your identity. Please be sure to bring identification documents when you visit the counter. The identification documents you need to present are as shown in the table below.
Note: Depending on the type of identity verification document, some require only one document to be presented (Category A), while others require two documents to be presented (Category B). Please be aware of this.

Documents with a photo attached issued by government agencies (1 item confirmation)
My Number Card, driver's license, driving history certificate, passport, identification card of government employees (including independent administrative agencies and special corporations) or documents equivalent to these

・Currently, our city's service counters do not support identity verification using the My Number Card (electronic certificate) installed on smartphones. When using the My Number Card as an identity verification document, please be sure to bring the original card with you.

B Documents proving identity other than A (2 points of verification)
Health Insurance Card, Health Insurance Qualification Confirmation Document, various Pension Certificates (passbooks), cash cards, credit cards, employee ID cards, and student ID cards or equivalent documents, tax notices related to the tax items and fiscal year of the certificate you wish to obtain (only at the time of applying for the issuance of the tax payment certificate)

Note: Identification documents with an expiration date must be within the valid period.

When a representative carries out procedures such as applying for the issuance of a tax payment certificate, a separate power of attorney must be submitted. The format of the power of attorney is free, but you can also download it from the "Power of Attorney" below, so please make use of it.
