Mobility Support Project

We provide mobility support (dispatch of guide helpers) for outings necessary for social participation, such as procedures at public institutions, essential outings for social life, and leisure activities.

Eligible Individuals

Those who meet any of the following criteria

• Holders of the Love Handbook or Mental Disability Health and Welfare Handbook
• Persons of school age and older

Usage Restrictions

The following cases are not eligible for use.

1. During year-round and long-term outings such as commuting (however, this excludes cases where the individual requires accompaniment for school and the Parent or Guardian is unable to accompany them).
2. When going out for economic activities such as business.
3. When going out for services specified in Article 8, Paragraph 2 of the Long-Term Care Insurance Act.
4. When going out for medical visits.
5. Other outings deemed socially inappropriate.

Usage Hours

• 18 years old and above, 25 hours per month
• Under 18 years old, 15 hours per month

Required Documents

Please submit the documents below, and you will be able to use them after the specified instructions are given.

• Application Form for Designation of Mobility Support Service Business Operators
• Copy of the designated notification issued by Tokyo Metropolitan Government
• A copy of any of the following documents that can prove
  • Certified Care Worker
  • Home Care Worker Training Course Level 1 or Level 2
  • Completion of the Initial Training Course for Care Workers (Basic Training Course for Care Workers, Home Helper Level 1 to 2 Course)
  • Completion of Training for Care Workers Providing Mobility Support for Individuals with Intellectual Disabilities (Limited to cases of providing mobility support for individuals with intellectual disabilities)
  • Certificate of Each Service

Regarding the Invoice Amount

• Personal care available: 1 unit 1,500 yen
• No physical care: 900 yen per unit

For those with physical care, it is printed on the recipient certificate.
