If you are newly using after-school day services and do not possess a Disability Certificate (Ai no Techo or Mental Health Certificate), we ask that you submit a doctor's Opinion Statement by regularly encouraging medical consultations. This will help to appropriately understand and confirm the condition of the disability and the necessity for developmental support at the right time, leading to appropriate assistance.

In cases where disability services are switched (e.g., from Child Development Support to After-School Day Services), individuals who do not possess a Disability Certificate (Ai no Techou or Mental Health Certificate) will need a physician's Opinion Statement.

Note: Those who possess a physical disability certificate may not need a physician's opinion statement, so please contact the Disability Welfare Division in advance.

Form

Considering the time from reservation to consultation and the time from consultation to the issuance of the Opinion Statement, it may take several months, so we kindly ask you to respond as soon as possible.

  • Diagnosis (including suspected cases), developmental challenges, etc.
  • Indication of the need for rehabilitation
  • (If possible) Results of developmental tests, etc.

Note: Please check if it is possible to issue an Opinion Statement with the above information when making a reservation at the hospital.

Q&A Regarding the Submission of Physician Opinion Statements

Question 1: I am not regularly visiting a doctor regarding developmental status. Where should I request a doctor's Opinion Statement?

Information about medical institutions related to developmental disorders is available on the Ministry of Health, Labor and Welfare's medical information network 'Navi'. Please make use of it.

If a referral letter is required for the examination, please consult your primary care physician.

If you are in trouble, please consult the Disability Welfare Division.

Note: Considering the time from reservation to consultation and the time from consultation to the issuance of the Opinion Statement, it may take several months. Additionally, some medical institutions may not issue an Opinion Statement without multiple visits, so we kindly ask you to take action early.

Question 2: It seems that I might miss the submission deadline to the city. What should I do?

In principle, the decision to provide services is based on the physician's Opinion Statement, but if you are approaching the submission deadline, please consult the Disability Welfare Division in advance.

Question 3: Why is a doctor's Opinion Statement only required when using after-school day services, rather than for child development support?

When children reach the age to enter elementary school, developmental challenges become somewhat clearer compared to preschoolers, and it becomes possible to make more appropriate assessments regarding service utilization. Therefore, we request the submission of a doctor's Opinion Statement when using after-school day services.
