Flow of Procedures for Disability Welfare Services
The process for receiving Disability Welfare services is as follows.
(1) Consultation/Application
When applying for benefits, please consult directly with the Business Operators you plan to use in advance, and submit the necessary documents to the Disability Welfare Division.
Regarding the service utilization plan draft, there is a "Service Utilization Plan" created by the local planning consultation support office, and a "Self-Plan" which is a service utilization plan draft created by the individual. Please submit one of these. For details on the required documents, please check the list of Application Forms below.
If you have any problems, do not know what services to use, or want to use new services, please consult with a disability consultation support business operator (Inagi City Social Welfare Council, Marche Inagi) or the Disability Welfare Division.

(2) Certification and Investigation (for those who need it only)
Certification of disability support classification will only be conducted for those who utilize nursing care benefits (home care, short-term admission, facility admission, etc.).
Certified investigators will conduct interviews regarding the condition of disabilities, intentions for use, living environment, and more through visits and other means.
Excluding children, the primary physician will be requested to provide an Opinion Statement from the Disability Welfare Division. Based on the information gathered and the physician's Opinion Statement, the disability support category will be determined at the Disability Support Category Assessment Committee, and the certification results will be sent by mail.
(3) Payment Decision
A certificate for recipients will be issued, detailing the type of service, the duration of benefits, and the amount to be borne by the user.
(4) Contract
Present the recipient certificate and directly conclude a usage contract with the business operators you will be using.
(5) Start of Service Use
Please use the services within the scope of the details listed on the recipient certificate, in accordance with the service utilization plan.
After use, users will pay the user burden amount and actual costs directly to the designated business operators/facility.
The recipient certificate indicates the validity period for each service. If you wish to continue using the services, a renewal application is required.
