Individuals who do not have certification for long-term care or support

Please consult with the responsible Community General Support Center.

Individuals certified as needing support level 1 or support level 2

Please consult with the responsible Community General Support Center.

Individuals certified as requiring care level 1 to care level 5

Please consult your care manager (certified care manager).
If you do not have a designated care manager, please consult the Community General Support Center in charge.
