QuestionWhat should I do if there is no primary physician when applying for Long-Term Care Insurance certification for care needs or support needs?

Response

It is desirable to specify a doctor who has treated injuries or illnesses that serve as the trigger for applying for long-term care or support certification, or a doctor who is familiar with the individual's daily life due to regular visits. However, if such a doctor is not available, please choose a new attending physician and visit a medical institution.
When visiting, please fill out the "Questionnaire for the Preparation of the Opinion Statement by the Attending Physician" and bring it with you to hand directly to the doctor.
At that time, be sure to inform them that you have applied for long-term care or support certification.
