Care plan verification extracted by home care support service office unit
From October 1, 2021, we will verify care plans that can lead to the provision of home care services that match the user's intentions and conditions, particularly those with a high utilization ratio of the category-specific payment limit and where home care constitutes a large part of the services used. If it falls under the standards set by the Minister of Health, Labor and Welfare (Notification No. 336 of the Ministry of Health, Labor and Welfare for the fiscal year 2021) and if requested by the city, please submit the relevant care plan to the city as a Notification. Please be fully aware that this system is not intended to restrict the use of services.
Applicable Home Care Support Service Providers
Home care support service offices where the usage ratio exceeds 70% of the category-specific payment limit and where more than 60% of the services used are home visit care.
Method of Verification
- The city requests the submission of care plans that correspond to home care support offices.
- Home care support offices examine the appropriateness of the care plan, describe the reasons for the need for home care in the care plan, and submit it to the city.
- The city will review the contents of the care plan and discuss the care plan at regional care meetings as necessary.
- If it is deemed necessary to review in the discussion, the home care support office will also reconsider the relevant care plan and care plans with similar or related content.
