About the Care Worker Treatment Improvement Allowance for Fiscal Year 7 of Reiwa
Deadline for Submission of Care Worker Treatment Improvement Plan
Regarding the care worker treatment improvement addition for fiscal year 2025, the submission deadline for the plan is usually set for the end of the month two months prior to the month in which the addition is obtained. However, the Ministry of Health, Labor and Welfare has issued a revised format for the plan as follows. Accordingly, if the addition is obtained in April or May 2025, the submission deadline for the plan will be exceptionally set to April 15, 2025 (Tuesday).

Regarding the termination of the transitional measures for the treatment improvement addition V(1) to (14)
The treatment improvement addition V(1) to (14), established as a drastic change mitigation measure due to the reorganization and integration of the treatment improvement addition, will end on March 31, 2025. After April 1, 2025, it will not be possible to calculate additions V(1) to (14), and facilities calculating addition V must change their calculation category and submit an addition notification, so please be aware.
Subsidies for Long-Term Care Insurance Business Expenses (Projects for Securing Care Workers and Improving Workplace Environment, etc.)
This is a subsidy for facilities that have obtained the Care Worker Treatment Improvement Addition and are working towards improving productivity, to assist with necessary costs for improving the workplace environment or labor costs.
The implementing body for the support subsidy is Tokyo Metropolitan Government. If you wish to apply for the subsidy, please submit "Appendix Form 2-3" and "Appendix Form 2-4" to the Tokyo Metropolitan Government.

Implementation of Projects for Securing Care Workers and Improving Workplace Environment
In addition, to respond to inquiries from nursing care service providers and facilities related to the implementation of this project, the following Ministry of Health, Labor and Welfare call center has been established.

Businesses that need to submit to Inagi City
Community-based service offices designated by Inagi City (including community-based day care service offices deemed designated)
(For business offices located outside the city, if you submit a notification of treatment improvement addition to the local government where the office is located, you may submit the same form with the addressee changed to the Mayor of Inagi City.)
- For facilities that are currently calculating the existing addition in fiscal year 6, if they plan to continue the calculation in fiscal year 7, it is necessary to submit a new plan for fiscal year 7.
- For community-based service providers located outside the city, if the use by insured persons of Inagi City has already ended, submission of the plan is not required. Please promptly submit the notification of the business closure.

Note 1: For the "Appendix Form 2 (Subsidy and Additional Plan Integration Form)", please submit the plan for the fiscal year 7 to Inagi City using "Appendix Form 2-1" and "Appendix Form 2-2".
Regarding "Appendix Form 2-3" and "Appendix Form 2-4", these are application forms for the Long-Term Care Insurance business expense subsidy (for securing nursing care personnel and improving workplace environments), so please submit them to Tokyo Metropolis.
Note 2: If a notification regarding changes or special circumstances is required, please submit "Appendix Form 4 (Notification of Changes)" or "Appendix Form 5 (Notification of Special Circumstances)" to the Mayor of Inagi City.
About the Additional Payment Notification
When newly calculating additional benefits, when the calculation category changes, or when the calculation ends, it is necessary to submit the additional benefit application along with the "Notification of the System Related to the Calculation of Nursing Care Benefits" and the "List of Conditions Related to the Calculation of Nursing Care Benefits" including "Appendix 1-3" and "Remarks (1-3)".

Submission Deadline for Plans and Additional Reports
Both the plan document and the additional payment application must be submitted by the end of the month before the month in which the additional payment is to be received.
However, the submission deadline for the plan from April or May of the 7th year of Reiwa will be set exceptionally on April 15, 2025 (Tuesday).
In addition, if a new addition is calculated from April or May of the 7th year of Reiwa, if the calculation category changes, or if the calculation ends, the deadline for submitting the addition notification will be exceptionally set to April 15, 7th year of Reiwa (Tuesday).
Note: If your business cannot submit by the deadline, please let us know in advance.

Submission of the Plan
Please fill in the required information in the Logo form at the URL below, and submit the attached file with the Excel file name formatted as [Start Month_Improvement Plan - Organization Name].
Submission of Additional Payment Notification
From April 2024, submission via the electronic application notification system for "Notifications Regarding Additional Charges" will be the standard. Please submit the "Notification Form Concerning the System for Calculating Nursing Care Benefits" and the "Status List of the System for Calculating Nursing Care Benefits" data as attachments.

Documents to Prove the Notification Details
When submitting the plan, the documents that serve as the basis for the contents and the following documents do not need to be submitted to the city, but must be properly stored, and must be submitted promptly if requested by the city.
- Work Rules as Stipulated in Article 89 of the Labor Standards Act
However, if regulations concerning wages, retirement allowances, temporary wages, career path requirements related to appointment conditions, wage system regulations, and the mechanism for salary increases are created separately from the work rules, those regulations are also included. - Documents confirming enrollment in labor insurance

Points to Note
When submitting documents, please carefully read the latest Long-Term Care Insurance information, Q&A, explanatory materials for additional calculation, and sample entries before submission.
When submitting a notification for multiple business locations collectively, if locations outside the jurisdiction of Inagi City are included, it is also necessary to notify the insurer responsible for those locations.
Consolidation of the Treatment Improvement Addition for Fiscal Year 2024
Due to the revision of nursing care fees for the fiscal year 2024, starting from June 2024, the "Nursing Staff Treatment Improvement Addition," "Specific Treatment Improvement Addition for Nursing Staff," and "Support Addition for Base Up for Nursing Staff" (hereinafter referred to as the "current additions") will be consolidated into the "Nursing Staff Treatment Improvement Addition" (hereinafter referred to as the "new addition").
- The "current addition" and the "new addition" are collectively referred to as the "Improvement Addition for Care Workers, etc.".

About Performance Reports
Businesses that have calculated the additional amount are required to submit a performance report. Businesses that have been discontinued during the fiscal year are also required to submit a performance report.
Submission Deadline for Performance Reports
Until the end of the second month following the month in which the final additional payment from the National Health Insurance Federation was received in each fiscal year
(Example: If the service was provided in March, the payment will be made in May, so the deadline will be the end of July)
