Regarding Claims of Errors in Long-Term Care Insurance

In the review by the National Health Insurance Association (hereinafter referred to as "Kohoren"), if there are errors in the claims for already determined Long-Term Care benefits and comprehensive business expenses, it will be necessary to submit a statement of error.

Please attach and submit the "Inagi City Error Report" file with the required information from the URL below: "[Inagi City] Long-Term Care Insurance Error Report Submission Form (External Link)".

Method by bringing to the counter or by mail

 Please fill out the necessary information on the "Inagi City Error Declaration Form" available under "Form Download" below, and bring it to the counter below or send it by mail.

Submission Deadline

Submit via the form by the 20th of each month, or if submitting in person or by mail, it must arrive by the 20th
(Note) If the 20th falls on a Saturday, Sunday, or public holiday, it will be the preceding weekday.

(Note) Submissions made after the deadline will be treated as submissions for the following month.

Notes

  • Please submit for each Business Operator. Even if the business office number is the same, please create and submit separately if the types of services are different.
  • Please prepare and submit the nursing care benefit costs and comprehensive business costs in their respective formats.
  • Please do not use the forms for the Disability Welfare service-related error claims.
  • Please do not forget to select and fill in the item "Re-invoicing: Yes or No".
  • For the reasons for the application code, please refer to the "Inagi City Long-Term Care Insurance Error Application Code Table."
  • Claims for errors will only be accepted for those that have completed the review by the National Health Insurance Association. If the claims for the previous month's services or those that have been returned are not finalized, you cannot file for an error, so please check the results before making a claim.
  • If there are items that require a resubmission within the error claim, please resubmit to the National Health Insurance Association with the correct details by the 10th of the month following the claim month (same month error). If you do not withdraw the actual results and do not resubmit, please only make the error claim. (Note) If you make a same month error, a difference adjustment will be made between the canceled amount and the correct amount.
  • If there are many cases of erroneous claims, we will adjust the submitted documents and submission methods, so please contact the responsible department in advance.
