QuestionPlease tell me about the name change procedure for National Pension subscribers and beneficiaries.
Response
Category 1 Insured Person
If your name changes due to marriage or other reasons, you need to complete the name change procedure at the Citizen Affairs Division. No procedures are required at the Insurance and Pension Division.
Note: Please notify your workplace or your spouse's workplace of any name changes for Type 2 and Type 3 insured persons.
Pension Beneficiary
Submission of the "Pension Beneficiary Name Change Notification" is required.
The application form is available at the Pension Office and the Insurance and Pension Division of City Hall.
We will issue a new Pension certificate after the name change, so please attach and submit all of your current Pension certificates.
Also, please complete the name change procedure with the financial institution where you receive your Pension.
Note: If you are receiving a survivor's Pension and your name has changed due to marriage or adoption, you will lose your right to receive the survivor's Pension. For more details, please check with the Pension office.
