QuestionPlease tell me about the procedures for when a Pension certificate is lost.

Response

If you lose your pension certificate, you will apply for reissuance at the Fuchu Pension Office.
Reissuance can also be done by mailing the Application Form for Reissuance of the Pension Certificate.

Items to Bring

Documents that show the basic pension number and pension code, along with identification documents (My Number Card, driver's license, etc.)
In the case of a representative, a power of attorney and identification documents of the representative (My Number Card, driver's license, etc.)
