If you lose your pension certificate, you will apply for reissuance at the Fuchu Pension Office.
The Pension Certificate reissuance Application Form can also be submitted by mail for reissuance. The form is also available at the City Hall Insurance and Pension Division.

Items to Bring

Documents that show the basic pension number and pension code, along with identification documents (My Number Card, driver's license, etc.)
In the case of a representative, a power of attorney and identification documents of the representative (My Number Card, driver's license, etc.)
