QuestionPlease tell me the procedures when a pension recipient passes away.
Response
When there is a Pension remaining to be paid to the deceased, the unpaid Pension will be paid to the surviving family members who were living together.
Notification Window
If a person receiving a pension passes away, please submit the Notification of Death of Pension Recipient along with the Application for Unpaid Pension to the Fuchu Pension Office or the Pension Section at City Hall (the application forms are available at the counter).
- Note: If you are eligible to apply for the Survivor's Welfare Pension, the procedure will be at the Fuchu Pension Office.
- Note: If the deceased was a member of a mutual aid association, it may be necessary to submit documents to each mutual aid association.
Necessary Items
- Family Register
- If the applicant is in the same family register as the deceased
Copy of the applicant's family register (after the date of death, showing the relationship to the deceased) - If the applicant is in a different family register from the deceased
Combination of the "applicant's family register" and the "deceased's family register" (documents issued after the date of death, showing the relationship between the two)
- If the applicant is in the same family register as the deceased
- Certificate of Residence of the applicant (must be issued after the date of death, containing information on the applicant and all household members, and must include "registered domicile," "head of household," "relationship," and "changes")
- Certificate of Residence removal for deceased persons (issued after the date of death, including registered domicile and relationship)
- Application for Co-habitation Relationship (When the claimant is in a different household from the deceased)
- Depositor's Savings Passbook
- Pension and Gratuity Certificates for Deceased Persons
- Identity verification documents of the applicant (My Number Card, driver's license, etc.)
- Note: If the applicant is the spouse of the deceased, by entering the applicant's My Number, items 1, 2, and 3 can be omitted.
- Note: If the applicant is not the spouse of the deceased, the applicant can omit 2 and 3 by entering their My Number.

- Spouse
- Child
- Parents
- Grandchild
- Grandparents
- Siblings
- Relatives within three degrees of kinship other than these individuals
When proceeding with the application, the requester must provide identification documents (My Number Card, driver's license, etc.), and in the case of a representative, a power of attorney and identification that verifies the representative's identity are required.
